Use this order form to add a special inscription to your existing Lynda Carter order.
Dedications are limited to two words only. Example: “To John.”
Quotes are limited to five words, but you may purchase multiples. The cost of the quote option is per five words. For each five words, you must add one to the quantity of this product. (for example, if your quote is 15 words long, you must purchase 3x this product.)

All sales for send-in items are NON-REFUNDABLE except in the event of an event cancellation. In the event of an event cancellation (not a re-scheduling of the event due to changing conditions or talent conflicts with a specific date), we will issue a refund within 4 weeks of the event cancellation.
